Once you have initiated your checkup using a valid pre-auth number, you will be sent an email with guidance on setting a password for your account.

If you are a Vitality member: If you have been referred to our service, you will need to follow the guidance in the email to initiate your checkup using a valid pre-auth number. Please enter VH into the pre-authorisation field followed by the date of your Vitality GP consultation in the following format: VHDDMMYY. Once you have initiated your checkup using a valid pre-auth number, you will be sent an email with guidance on setting a password for your account.
