  1. Which organisation are you from?
  2. Brief history of the issue - What exactly is occurring, when did it start etc.
  3. If the issue relates to a case, send us the checkup ID number(s)
  4. What device are you having issues with?
  5. Are you using mobile data or wifi or both?
  6. Send us a screenshot of the issue if possible
